Office Policies

Welcome to Walton Eye Care. We realize you have a choice for your eye care and we are pleased you have chosen us to provide your care.

It is very important that you notify our receptionist of any address changes, phone number changes, or change in insurance before you are seen.

Appointments

Call our office at (770) 267-4561 during normal business hours and someone will be happy to assist you in scheduling an appointment that is convenient for you.

You can also fill out our online Request an Appointment form. A representative from our office will call you within 48 hours to confirm your appointment.

Cancellation Policy

Please call us at least 24 hours in advance to cancel or reschedule your appointment.

Running Late

If you are running late let us know. We may be able to see another person ahead of you or reschedule you for another day. We value your time and we take pride in our efficiency.

Check-in

If you are scheduled for an appointment, arrive at least 15-minutes ahead of your scheduled reservation so we can complete all paperwork before your appointment time. Don’t forget to put your cell phone on vibrate or off during your visit.

Request for Medical Records

Please allow 48 to 72 hours to process this request. Call us at (770) 267-4561.

Prescription Request

Prescriptions and refills for patients of Walton Eye Care are routinely issued during normal office hours.

Please allow 24-48 hours (excluding weekends) for your prescription refill request to be processed. Once you have submitted your request and allowed processing time, please check with your pharmacy that your prescription has been filled prior to contacting our office. We ask that you request your medications at least 3 to 4 days before your prescription is due to run out.

Clinical Questions

Please be aware if you call our office with a clinical question, our Physician and Medical Assistants are in clinic during the day and cannot be called away from patients to speak to you. Our receptionist will get your message to our clinical staff and they will return your call as soon as possible.

NOTE: if you have recently had surgery, please notify our receptionist of any problem you are experiencing and she will immediately notify a member of our clinical staff.

Billing and Payment

Payment

In order for our office to submit an insurance claim, you are required to present your insurance information and valid photo identification at the time of check-in. You are responsible for any co-payment or deductable at each visit, in accordance with your insurance policy.

The statement you receive from our office includes only Walton Eye Care fees. You may also receive separate bills from the hospital or other physicians.

If you do not have insurance, payment is due in full at the time services are rendered. Please check with our office about payment plan options. We accept cash, check, debit card and most major credit cards.

Accepted Insurance

We participate with and accept most insurance plans including Medicare and Medicaid; however, it is the responsibility of the patient to ensure that Walton Eye Care is covered under his/her plan. For current information regarding your insurance, please call our Eligibility Department at (770) 267-4561.

Referral Authorization

Some insurance plans require the patient to obtain a referral authorization from their primary care provider prior to receiving services from a specialist. If your plan requires a referral authorization, services will only be provided if the referral authorization is at our office at the time of your appointment. You will have the option to sign a waiver accepting financial responsibility for services not covered due to the lack of referral authorization. If referral authorization is an issue, we can reschedule your appointment.
